Case Management

APNH Case Managers are available to provide services to individuals living with HIV/AIDS and their families in the greater New Haven area. The Case Manager participates on an Inter-agency Case Management Team, coordinated by APNH's Director of Client Services, working with clients to assess their needs, develop comprehensive long-term care plans, and broker services.

Our goal is to provide quality services and to link our clients to available resources in the community. Our services include:

• Assistance with financial entitlements
• Assistance with housing concerns
• Assistance with medical, emotional, and social needs
• Referrals to other community and state agencies
• Support group
• Emergency financial assistance
